A sagging gutter can usually be re-hung with new hangers if the gutter itself isn't bent and the fascia board behind it is sound. Replacement becomes the answer when the run is kinked, the metal has fatigued at the seams, or the fascia is rotted — in that last case the fascia has to be fixed first or the new hangers won't hold either.

If a clean gutter still overflows, the usual causes are incorrect pitch (water pools instead of flowing to the outlet), an undersized system for the roof area feeding it, or a clogged underground drain the downspout empties into.
